Flu Vaccine 2010 Compliancy Requirements and Clinic Schedule
To become compliant for seasonal flu please do one of the following:
- Get vaccinated! Flu clinics are located in the SHC Atrium Passageway M-F from 0600-2000, Oct. 4th – Dec. 3rd, 2010 View Flu Shot Clinics Schedule – includes offsite locations
- Provide documentation of vaccination elsewhere; Find the forms on the intranet under departments/occupational health/flu page
- Refuse vaccination; Take appropriate Healthstream course by searching through the Healthstream catalogue for 2010-2011 Flu Vaccination Refusal
